How to join:
Please print and fill out the application (pdf 51.56 kB) (attach all required documentation attached) with an original signature, and return to the Agency Relations Director. You will be contacted by the Director to schedule a new agency orientation as well as a pre-site monitor visit prior to approval of your site. Once you have been approved, you will be able to start ordering/shopping for your clients.
